How to Get a Ketamine Referral Through the VA

The VA covers ketamine treatment for eligible veterans through Community Care. Here's exactly what to say, which criteria to meet, and how to advocate for yourself.

Schedule Your Appointment

Make an appointment with your Primary Care Provider (PCP), Psychiatrist, or any established provider at the VA.

Pro Tip

If you already have care established at a Community Care office, that's even better—they handle all ketamine referrals directly.

Request the Referral

At your appointment, specifically request:

Ensure You Meet the VA Criteria

Your provider must document that you meet these requirements in the referral (per the official VA National Protocol):

  • Qualifying Diagnosis Treatment-Resistant Depression (TRD) with moderate to severe symptoms, documented by a VA psychiatrist or licensed Mental Health provider.
  • Treatment History Requirements 2 antidepressant trials (including one augmentation strategy such as therapy) in your current episode, plus 4 total adequate trials in your lifetime. Each trial must be at least 6 weeks at half-maximum dose or greater. Have your provider document everything: medications, talk therapy, group therapy, etc.
  • PHQ-9 Score > 15 Score greater than 15 within the past 30 days, indicating moderate to severe depressive symptoms.
  • Informed Consent & Safe Transport You must provide signed informed consent and have a safe method of transportation arranged (you cannot drive yourself home after treatment).

If Your Provider Says "We Don't Do Ketamine"

Some providers aren't aware of the VA's ketamine protocols. You can show them the official VA policy document (linked below) to prove that ketamine treatment protocols exist under the National VA protocol.

The SEOC (Service Order) number for ketamine treatment is 1.10.11. This covers IV Ketamine and psychotherapy for 6 months at a time.

Committee Review

Once submitted, your referral goes to the SMILE and/or Somatic Committee, a multidisciplinary team that reviews ketamine consults once per month.

They'll review your cardiovascular history, medical evaluations, and behavioral health records. If approved, they'll sign off and fax the referral to us for treatment.

Timeline

This entire process can take a few months depending on how quickly the committee reviews your case.
