This Privacy Policy describes how Valor Health Solutions, LLC (“Valor,” “we,” “us,” or “our”) collects, uses, discloses, and protects
information in connection with your use of the website valorhealthconnect.com and related online services
(collectively, the “Site”).
This Privacy Policy applies to information collected through our public-facing Site. It does not replace, and is separate from, our
obligations under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) with respect to Protected Health Information (“PHI”).
If you are a patient or client, our permitted uses and disclosures of PHI and your rights under HIPAA are described in our
HIPAA Notice of Privacy Practices.

1. Relationship to HIPAA and Clinical Systems (PHI)
If you become a patient or client of Valor, certain information we maintain about you may qualify as PHI under HIPAA. PHI is stored and processed
in secure, HIPAA-regulated clinical systems (such as our electronic health record, patient portal, telehealth platforms, prescribing systems, and
related tools) that are separate from this Site. The public Site is not used as an electronic health record and is not intended for submitting
sensitive medical information.

This Privacy Policy addresses how we handle information collected through the Site that is not PHI or otherwise subject to HIPAA in the context in

address, phone number, city and state, preferred clinic location, and any other information you choose to provide in contact or inquiry fields.
Please avoid submitting detailed medical information through general contact forms. If you are an established patient, use the patient portal or
other clinical channels for clinical information.
2.2 Information Collected Automatically
When you visit the Site, we may automatically collect certain information about your device and use of the Site, such as your IP address, browser
type, operating system, referring URLs, pages viewed, and the dates and times of your visits. We may collect this information through cookies, web
beacons, log files, or similar technologies.

We partner with Microsoft Clarity and Microsoft Advertising to capture how you use and interact with our website through behavioral metrics, heatmaps, and session replay to improve and market our products/services. Additionally, we use this information for site optimization, fraud/security purposes, and advertising. PHI maintained in our clinical systems is subject to additional safeguards required by HIPAA and other applicable healthcare laws and regulations.
7. Data Retention
We retain information collected through the Site for as long as reasonably necessary to fulfill the purposes described in this Privacy Policy, comply
with our legal obligations, resolve disputes, and enforce our agreements. We may retain anonymized or aggregated information that does not identify
you for longer periods.
8. Your Choices
You may choose not to provide certain information through the Site, in which case some features or services may not be available to you.
If you receive marketing or promotional emails from us, you may unsubscribe at any time by following the instructions included in those emails or by
contacting us at privacy@valorhealthconnect.com. Even if you opt out of marketing communications, we may still send you non-promotional
messages related to your inquiries or transactions.
To request access to, correction of, or deletion of certain personal information collected through the Site, you may contact us at
privacy@valorhealthconnect.com. We will respond to such requests in accordance with applicable law. Requests concerning PHI should be made as
described in our HIPAA Notice of Privacy Practices.
9. Children’s Privacy
The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. If we
become aware that we have collected personal information from a child under 13 through the Site, we will take reasonable steps to delete it in
accordance with applicable law.
Clinical services provided to minors are handled through our clinical systems in compliance with HIPAA and applicable state laws concerning consent
and confidentiality.
10. State-Specific Rights
Depending on your state of residence, you may have additional rights regarding your personal information under state privacy laws. These may include
rights of access, correction, deletion, and restriction of certain processing activities.

of the revised Privacy Policy.
